Q3 1998 — Multi-Site Survey Summary

Third quarter survey operations across 47 active sites proceeded within expected parameters. A total of 1,204 individual descent operations were logged between July and September 1998, with a combined survey depth of approximately 318,000 meters across all standard-tier sites.

Notable completions include the finalisation of the Eastern Basin geological assessment contract (REF: GC-1997-12) and the commencement of preliminary surveys for three new infrastructure consultation engagements scheduled for Q4.

No significant personnel incidents were recorded across standard-tier operations during this period. One minor equipment failure was logged at Site 44-C and resolved within 48 hours. Full equipment log available to credentialed personnel via the Employee Portal.

Personnel Safety Protocol Update — Enclosed Environment Operations
SEP 18, 1998 PUBLIC SAFETY

Following a scheduled review of enclosed environment operating procedures, the following updates to standard safety protocols are now in effect for all field personnel. These updates apply to all standard and restricted-tier descent operations.

PROTOCOL UPDATE 1998-SP-04
All personnel conducting descents beyond 200m are now required to log instrument readings at 50m intervals rather than 100m intervals as previously mandated.

PROTOCOL UPDATE 1998-SP-05
In the event of instrument discrepancy during descent — including but not limited to distance looping, compass deviation, or temperature reversal — personnel are to cease descent immediately, mark the depth of anomaly in the field log, and return to surface. Anomaly reports are to be submitted within 24 hours of return.

PROTOCOL UPDATE 1998-SP-06
All field personnel are reminded that the buddy system is mandatory for all descents at RD-tier locations regardless of experience level. No exceptions.

Personnel with questions regarding updated protocols should contact their site supervisor or the relevant divisional safety officer.

Site RD-01 — Automated Telemetry Suspension Notice
AUG 22, 1998 ANOMALY FLAG RD SITE

Automated depth telemetry systems at Site RD-01 (Northern Ravine, Northern Territories) have been suspended as of August 21st, 1998, following a series of instrumentation anomalies logged over the preceding 72-hour period.

ANOMALY LOG — RD-01 — AUG 18–21, 1998
SUPERVISOR: J. MORRISON

AUG 18 — 22:14: Automated depth probe returned reading of 4,827m. Within expected range. Probe retracted normally.
AUG 19 — 03:41: Automated depth probe returned reading of 4,827m. Retraction took 4 minutes longer than logged descent time would indicate. Noted.
AUG 19 — 11:02: Second probe deployed for cross-reference. Returned reading of 4,827m. Retraction time normal.
AUG 20 — 00:17: Primary probe returned reading of 4,827m. Retraction initiated. Cable tension inconsistent with expected resistance profile for logged depth. Probe retracted. Equipment inspected. No fault found.
AUG 21 — 04:58: Primary probe deployed. No reading returned. Probe retracted. Cable length consistent with a descent of approximately 4,827m. Depth instrument returned blank.
AUG 21 — 05:03: Manual logging protocols initiated. Automated telemetry suspended pending review.

Manual logging is ongoing. Site access remains restricted to D-4 clearance and above. This report has been reviewed by the Depth Mapping Authority. Status: monitoring.

Q2 1998 — Eastern Basin Contract Completion Summary
JUL 14, 1998 PUBLIC SURVEY SUMMARY
RPT-1998-021

The Eastern Basin geological stability assessment contract (REF: GC-1997-12) has been completed and submitted to the client ahead of schedule. The assessment covered 340 square kilometres of subterranean formation across four survey zones, with a total of 88 individual descent operations conducted over 14 months.

Full findings have been delivered to the commissioning body. A public summary will be made available through the Survey Archive upon client approval, expected Q4 1998.

DM-Series Vehicle Fleet — Maintenance Cycle Report
JUN 30, 1998 PUBLIC EQUIPMENT
RPT-1998-018

Annual maintenance cycle for the DM-series descent vehicle fleet has been completed. All 14 vehicles in the active fleet have been serviced and returned to operational status. Three vehicles were removed from active rotation pending component replacement and are expected to return to service by Q4 1998.

The second-generation DM-series prototype vehicles commissioned earlier this year are currently undergoing field testing at designated evaluation sites. Operational deployment is expected to begin in early 1999.

Site RD-01 — Personnel Log Discrepancy — Resolved
MAR 04, 1998 ANOMALY FLAG RD SITE
RPT-1998-008

A discrepancy was identified between the descent log and the surface check-in record for Site RD-01 during the week of February 23rd, 1998. One personnel entry recorded a descent at 02:30 on February 25th that does not correspond to any scheduled operation for that date.

DISCREPANCY NOTE — RD-01 — FEB 25, 1998
SUPERVISOR: J. MORRISON

Unscheduled descent entry logged at 02:30. Duration: 6 hours 14 minutes. Personnel ID on log does not match any current active roster entry for Site RD-01.

Equipment audit conducted. No equipment reported missing or displaced. Surface access log shows no corresponding entry for the listed personnel ID.

Status: reviewed. No further action required. Log entry attributed to administrative error in personnel ID transcription. Resolved.

This report has been reviewed and closed by the Depth Mapping Authority. No further follow-up is required.
